Positive Behaviour Support Services in Brisbane

We tailor interventions and supports aimed at reducing behaviours of harm to self and others, and replace them with strategies to meet the individuals need in a positive way. Hand in Hand ‘Early Intervention Allied Health Services’ work closely with families, schools, and daycare providers to offer a holistic approach to behaviour support.

Hand in Hand ‘Teens Thrive Allied Health’ is a specialised service working with teenagers and support networks to build their capacity, independence and quality of life while reducing behaviours that cause harm to themselves or others. Hand in Hand's Adult Allied Health services work closely with individuals, supported independent living arrangements and aged care providers to identify behaviours of concern, functions of behaviours and implement strategies to reduce restrictive practice matters.

Our behaviour support services are here for the times when individuals,  families and carers feel unsure of what to do next. Our NDIS-qualified and registered Allied Health Practitioners work closely with individuals and their support networks to understand what’s driving the behaviour and develop practical, positive strategies that create meaningful, lasting change. We are here to help build daily life skills, restore confidence, and support each step forward.

What Is Positive Behaviour Support?

Positive Behaviour Support (PBS) is a person-centred, evidence-based approach that helps people understand and manage behaviours of concern in ways that improve quality of life. Rather than focusing solely on the behaviour itself, Behaviour Support looks at what the individual is trying to communicate or achieve, and supports them to build new skills, strengthen relationships, and feel more confident and connected in their daily life.

Who Benefits from Behaviour Support?

  • NDIS Participants who exhibit behaviours of harm to themselves or others to get their need met.
  • Families, carers, and support workers looking for strategies and support to implement replacement behaviour strategies.
  • Rather than simply reacting to behaviours, behaviour support proactively addresses the function through structured, personalised intervention plans developed by qualified behaviour support practitioners.

How Can We Help?

A Positive Behaviour Support Practitioner works closely with individuals, families, and support networks to author behaviour support plans, develop and implement positive strategies that promote positive change, reduce and eliminate restrictive practice matters.

Our team of experienced and qualified Behaviour Support Practitioners at HandinHand Mental Health & Disability use a data-driven approach to help individuals understand, manage, and improve behaviours for a better quality of life.

PBS Assessment

Functional Behaviour Assessments

Understanding the root causes of behaviours is the first step toward meaningful change. Our behaviour practitioners conduct comprehensive functional behaviour assessments to identify triggers, behaviour patterns, and underlying reasons contributing to challenging behaviours.

These assessments provide the foundation for developing effective, person-centred strategies tailored to the individual's unique needs and circumstances.

PBS Skills

Skill Development Planning and Implementation

Positive Behaviour Support is not just about managing behaviour. It is about building practical skills that support daily life and build capacity.

We develop personalised plans to help individuals develop effective communication skills, emotional regulation, and social interaction. These skills replace challenging behaviours with more positive, sustainable alternatives, and support long-term independence.

PBS Training

Education and Training

Supporting behaviour change requires a collaborative approach. We provide education and training to families, carers, and support staff throughout Brisbane to ensure everyone involved has the knowledge and tools to implement Positive Behaviour Support strategies effectively.

Our training sessions cover:

  • Understanding behavioural triggers
  • Proactive strategies to reduce challenging behaviours
  • Effective communication techniques
  • Crisis management and de-escalation methods

Through ongoing guidance and support, we empower carers to create a safe, structured, and positive environment for the individuals they support.

Behaviour Support Services in Brisbane

We support individuals with practical strategies that reduce challenging behaviours and promote independence. Our experienced and qualified Behaviour Support Practitioners work across Brisbane to deliver plans that aim to improve communication, emotional regulation, and social skills.

Positive Behaviour Support Plans (PBSPs)

Every plan is built around the person. We create PBS plans with:

  • Proactive Strategies
  • Reactive Strategies
  • Crisis support techniques
  • Replacement behaviour strategies

All plans align with NDIS Guidelines and are developed in collaboration with the individual,  families and their support teams.

NDIS Funded Support

We provide PBS services under the ‘Improved Relationships’ funding category and assist with eligibility, planning, and reviews, whether you are self-managed, plan-managed, or NDIA-managed.

If you don’t have Improved Relationships Funding in your NDIS Plan, reach out to ask us about providing an ‘Assessment and Recommendation Report’ to inform potential funding under Improved Relationships.

Ongoing Support

Positive Behaviour Support is a process, not a quick fix. We offer regular check-ins, plan reviews, and hands-on support to make sure strategies continue working as needs change.

Behaviour Support requires a level of non-face-to-face work in order to develop and author the Behaviour Support Plan, and the creation of Strategies.

Supported Independent Living Provider Support

We work with Supported Independent Living providers to support individuals with;

  • Functional Assessments
  • Behaviour assessments
  • Behaviour Support Plans tailored to the person
  • Identify any environmental impacts on the person and provide recommendations
  • Provide Staff training in proactive, reactive strategies, understanding the function of behaviour
  • Least restrictive alternatives
  • Reduction and elimination of restrictive practices

Creating A Path To Positive Change: Your Personalised Behaviour Support Journey

Our approach is built on collaboration, understanding, and ongoing support to help create meaningful, lasting change for Brisbane families.

How We Guide You Through the Process

Step 1: Referral and Intake

When you call 1300 853 293, you are not just making an enquiry, you are beginning a new chapter. We'll take the time to understand your situation, listen to your concerns, and explore how we can best support you or your loved one in Brisbane.

Service Agreement and Getting Started

Once we have discussed your support needs, we can guide you through a Service Agreement. This is a straightforward document outlining the services we will provide and what you can expect from us. This ensures clarity and sets the foundation for a strong partnership.

Meeting Your Behaviour Support Practitioner

Following the intake process, we will introduce you to a dedicated Behaviour Support Practitioner. This professional will work closely with you, offering guidance and practical strategies to support positive change.

Step 2: Collaboration and Stakeholder Engagement

Positive Behaviour Support works best when everyone is on the same page. Our Practitioners will connect with your family members, support workers, Support Coordinators, and other key individuals, ensuring a holistic approach to your care.

Understanding Your Story

We take the time to learn about your experiences, challenges, and goals. By listening to both you and your support network, we gain a deeper understanding of your needs, allowing us to develop strategies that truly reflect your unique situation.

Step 3: Drafting a Positive Behaviour Support Plan

Using insights gathered from our discussions and assessments, we create a Positive Behaviour Support Plan (PBSP) tailored to your needs. This plan serves as a structured plan and strategies designed to reduce challenging behaviours and foster positive change.

Identifying Behaviour Triggers and Strategies

We explore the underlying reasons behind certain behaviours, identifying triggers, patterns of behaviour, and unmet needs. From there, we develop practical, person-centred plans and strategies to address challenges, promote skill development and reduce restrictive practice matters.

A Thorough, Well-Considered Approach

Creating an effective PBSP takes time. Over a 3 to 6-month period, we carefully assess, refine, and adjust the plan to ensure it aligns with your needs and long-term goals. This thorough process ensures that the plan is not just effective but also practical and achievable.

Step 4: Implementation and Training

Once your PBSP is finalised, our team will guide you, your family, and support workers through the implementation process. We break it down into manageable steps, ensuring that everyone involved feels confident in applying the strategies.

Ongoing Support and Adjustments

Behaviour support is an ongoing journey. As you begin to use the strategies in everyday life, we remain actively involved, reviewing progress, celebrating milestones, and making necessary adjustments. Your PBSP is not a static document; it grows with you, ensuring continued success.
